It's not about what we think. It's about people who work at ratings think. And they've increased ratings for far less. If a character isn't portrayed as smoking in a negative way (by them being a villain), or if the act of smoking isn't explicitly viewed as bad (like having your character cough and cartoonishly throw it away), it can be perceived as glamourized.

For further examples, check out the Hays code. It's not the same, but it's similar in its execution: if the action exists, it must be portrayed as either negative or have connotations around it that show it's an immoral act, such as being associated to immoral characters.
