I've had this happen at level 60. The SMN apparently thought I wasn't going to cure him when he had 6k HP remaining in Fractal. What I was really doing was finishing casting my Holy/Assize rotation so I could pop out of Cleric's and Tetra him, since no AOE was coming and he wasn't doing enough damage to take hate.I am new-ish but I'm loving this game with a passion and healing is something I'm enjoying a lot. Anyway, I don't have many horror stories... but there's one thing I will remember.
Me: *stares at the hp of the tank because we don't want to overheal*
Arcanist: Don't worry, I WILL HEAL!! *starts spamming cure attracting all the hate*

Yet there he was, curing himself.
Here is mine:
Levelling my little Scholar I get Copperbell Mines with a DRK, PUG, and BRD. Everything is going great... I am dpsing and letting my fairy do all the healing duty.... UNTIL... we blow the wall after the second boss and the tank decides it would be a brilliant plan (with no warning mind you) to sprint (literally.. he used sprint) to pull _everything_ up until the final hall for the boss. Welllll... needless to say he had hate on... well.. probably nothing so Ms. Fairy decides it would be a good idea to take a dirt nap. Now her friends are eating MY face off. Tank manages to get all but 2 or 3 off me but obviously I can not take time to resummon my fairy so I am spamming Physick as fast as the GCD will let me. Tank eventually dies because I am having to move near constantly from various aoes going everywhere because of exceptionally poor positioning (and things still nomming on my face). Everything the tank has now decide it is time to play with the BRD... eh we can do this as long as the bard kites in circles til the tank gets back. Pshh, no way.. the BRD decides he should take off running.. somewhere.. I don't even know (side note: I would really like to know what is going through DPS minds when they run away from the healer.. it's not just here..it's all MMOS.. sometimes you get the good players that realize "if I stay close to the healer I have a chance of living but if I run away I am going to die" and then you get the ones that must be like "AAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAH RUN AWAY!". Anyway BRD goes down and everything else decides to come play with me and the buddies I have had for a while now and I go down. Apparently PUG is familiar with the art of kiting so he manages to stay alive waiting for the rest of us to get back. No one says anything, we finish everything up and go kill the boss but... wow.

Had coppebell HM too last week. The tank and smn were new. The melee and I weren't. The tank was exceptionnally squichy, despite a not too bad gear and we made it to the last boss just fine.
Last boss made us wipe 4 times. Tank was too squishy to keep alive while I had to feed the worms at the same time. The SMN was a bit slow at killing adds -if trying at all- and the melee (an experienced player) was sticking to the strat and killing boss only. The aggro was bouncing between the tank and melee and occasionnally to me. Having 3 tanks taking huge damage from both adds and boss, there's not much I could do, due to the sync.
Someone finally asked, should be abandon ? I said "No, please take your titan-egi. You (smn+ tank) take down the adds as they pop. I will heal Titan and feed the worm. Paladin, stay in sword oath, you'll be extra dps".
Cleared it no pb. In the end we gave a few advice to the tanks about aggro-ing. Got all 3 comms for saving the run. I LOVE TITAN.
A couple weeks ago I hit 60 on AST. Within a couple days I was in mostly full Alex normal gear due to lucky rolls, the rest was law gear.
Let me preface, I'm very critical of my own healing, however, I've never had a 4 man dungeon group wipe so completely that everyone abadons, nor have I ever been kicked for my healing. I'm very apologetic if something happens that I could have prevented.
I unlocked the arboreteum and queued up. I've run it countless times as Bard and am extremely comfortable with the mechanics of the place.
The tank I got unfortunately paired with did the big pulls, which I'm fine with, but he did not pop a single cd EVER. I'm not gonna lie, I struggled, HARD.
We do ok up to the part with the flower bears, he pulls like all the way up to the tree monster right before the last boss (after the gate lock). His health is yoyoing. I'm popping my cds like crazy, trying to get him up, but he's just getting pounded. Surprise, not a single cool down popped except the DRK oh shit I'm gonna die button (forget the name, I'm familiar with how it works).
We wipe, I couldn't keep up. He says something like "when you see the red icon you need to heal me to full or else I'll die". Thanks sherlock. I retort with "I'm trying, are you popping any cool downs or just waiting until you're about to die?" No respokse, but before I can even rebuff everyone he pills a group that wasn't killed.
We get to last boss, everyones getting owned by frond fatale, standing in things, not killing adds fast enough.
We wiped 3 times on this boss. I'm not perfect, but that's probably the worst tank I've come across. Ive never quit on a group before but I took the penalty and gave up on that one. When you make the same mistakes over and over with no improvement I just cant.

I had something similar just last night, only it was the healer who was being twat.
I was tanking Fractal. Near the end of the first mob the the Healer's Hp bar grey's out. No more heals. They disappeared. it was kinda strange and made me think they DC'd for a minute, but the mob was almost dead so it was no big deal. We move on, and we find the healer sitting patiently before the second set of Mobs. She has to heal me before the fight even starts because she ran off ahead of us. It's a bit unorthadox, but I can dig it.... only she choses to heal me with Regen.
She instantly pulls mob enmity as soon as I hit the first Mob, and I have to backtrack and waste a bit of Mp using an extra unleash or 2 to get enmity back (which was a pain because the healer was running away from me instead of to me). Positioning is a bit off and my Mp bar is wounded, but I get it sorted out. Healer disappears again. I'm seeing a trend. We kill the mob and follow her, and she casts regen on me to make up the Hp she wasn't there to heal. I don't want a repeat of the last pull, so I turn off Regen before I pull.... and she puts it back on just as I do the pull. She steal enmity right off the bat, again.
Next pull she does the same thing. I turn off Regen. She puts it back on. I turn it off, again and pull as fast as I can before she can reapply it, which she does as soon as she can. I'm about to tell her to knock it off, when she goes into the chat and says: "Stop turning my regens off! You're wasting my Mp." I look at her Mp bar. It's full. I look at mine. It's nearly empty because I had to chase mobs around while using Unleash to try and get aggro back from the mobs she insta-steals by keeping Regen up at the start of each pull.
it's refreshing to see healers (especially new-ish ones) who have a better understanding of how enmity works than lvl 60 players who do crap like this.
This comes from a different MMO I played years ago, but I think most healers can totally relate to it.
