The times we did it:

1) Returning the Scion's souls.
2) Returning G'Raha's soul (his was kind of different).
3) Bringing Unu to the First.

The time it was done to us:

1) Attuning to that watch thingy and bringing the WoL to the First (this also opened up a stable path through the semi-linked Aetherial Seas, but only for us since we existed in both in body as well as soul)

Portals to the Thirteenth: (Sorry I've forgotten their names)

1) When the boy (forget his name) was training with Ardbert's mage friend (as a boy) and they opened a rift successfully that sucked him in.
2) When the Sin Eater that was said mage friend was opening rifts of various sizes left and right trying to open one to save his friend.
3) When WE successfully rescue said friend through one.

Clearly, the Voidgates so opened allowed travel of people

Moreover, for all we know, what may be about to happen is just such a breach. That seems to be Golbez desire, anyway. And if you haven't talked to Cylia or Unu since 6.1, you should. They have a bit of commentary on the subject. I'm not certain if they were Memorates, but Cylia knows of them, and we know Unu has the Echo and was a Warrior of Light (albeit, a child one with little experience and power, too young to make much of a difference) as well. And we know what having the Echo means in general and being a Warrior of Light has some significance.
I didn't look any of them up. You have to be completely disjointed from the story to not know what an x.3 patch has been in the past. They are big finale patches except 2.3 which is more out of the norm for just being a build up and not a finale. Not the ops alt but just proving a point that the patch cycle has been well structured and we are in a giant filler phase.
