Nearly a decade ago threads like this popped up when newer gear began to ignore Miqo'te tails and the 1.0 tail holes (and amazing darklight set complete with tail armor that has only appeared again once or twice since) gave way to clipping through cloaks and male Miqo'te's default stance completely missing the holes that existed. SE didn't do anything then except for rare cases like the dragoon set in Shadowbringers, which also came at the same time as Miqo'te hat uniformity, so they won't do anything now.
At this point SE seems to be happy to be in a state of "clipping is a part of the game, unless you play a Viera or Hrothgar because their heads are weird and were purposefully made to not wear hats and so have horns or hair styles that will not work".
It's just wild speculation fueled by frustration on my part, but I don't think having the new races has done any favors for the old ones and in order to increase productivity and complete the expanded roster of armor sets/resizes, shortcuts were made and that's what leads us to Miqo'te hats missing ears in the present day. A particularly nice piece that pays direct attention to specific racial anatomy may still happen once in a blue moon when available time meets passion, but I don't see that happening as much. SE could hire more modelers, but that brings a cost and SE has already had years of existing clipping so there's no point in paying people to fix something that is mostly ignored by the larger playerbase.
This type of lazy shortcut process has made its way into the fabric of the game itself and probably isn't going anywhere. Au Ra concept art consisted of completely unique looking races that had horns on the tops of their heads (where one would expect) plus elf/faun ears or completely monstrous faces. But since that's hard and it's difficult to make them have their own thing, they got turned into Miqo'te/Elezen with a skin condition and ears that are somehow also horns along with a really weird sexual dimorphism that's all just handwaved in order to make a race that has tall, edgy, gruff men and short, idol-like women in order to appeal to the greatest demographic possible to justify the development costs, while also keeping costs down by homogenizing the designs.
If it really gets to you like it did to me after years of playing Miqo'te and then an Au Ra, you could just play a Midlander Hyur. Everything is specifically modeled to my body before being adapted to other races so I look good in everything and nothing clips except for hair styles (of which I have more than any other race in the game).
