Pretty sure the MSQ gives you level appropriate gear (and if you're lucky, dungeon dropping gear you need).

This depends on how you play. I always do all the side quests in an area the first time I'm there, I never skip anything. When I leveled up, my character's actual level was way ahead of the MSQ level, so my quest reward gear was always outdated by the time I got it.
