I main WHM this fight and we are currently into phase 3 making good progress. I can tell you what we do that allows us to consistently see phase 3. I am i125, I have my Zodiac i125, Cure 1 hits for 1500. It is key your tank rotates good cooldowns during this, our paladin is our MT and is great at using stoneskin on himself.
P1: My goal here is to never use cure 2 to conserve mana. Our scholar primarily dps's here. Aids me during the triple flare breath, earth shaker, and sacred soil during megaflare.
We follow Solitude's positioning guide and strat. I encourage you to look that up.
To start out, I always have regen on the tank. Every megaflare I medica 2 if divine seal is available I use it here. I let that tick and heal everyone up. I cure 1 the tank, regen, head back to my spot which is just to the right of Bahamut facing him. The SCH stands to the left facing him, both of us near the back. Flatten comes, our paladin stoneskins himself, I time a cure 1 to land right as it goes off, then start spamming cure 1 for the triple flare breath, regen at the end of the last one. After the 3breaths, Earthshaker is coming. Two people get marked, one goes to the right the other to the left. Since I am standing at the right, I heal that one. As they are running to drop their fire circles, I throw a regen on them and Cure 1, twice. Let the regen heal them the rest of the way and immediately cure 1 the tank for the upcoming flarebreath, regen the tank and get ready for megaflare again, rinse and repeat. We typically get 2-3 flattens and I go into P2 with near full mana. Note if the OT gets earthshaker, I just throw a regen on him and let him take the damage like a boss. We use cover on our monk if he gets it, the rest of our dps is ranged.
Gigaflare. I use shroud everytime it is up and don't save it for aggro reduction unless it is conveinent. Our OT is a warrior. Usually what ends up happening is that last megaflare, I already had a medica 2 rolling and it has about 10 seconds left on it as we transition from Giga into phase 2. I let this tick after the gigaflare and hold on AoE heals until the the add is dead, eye for an eye on the OT, virus on the add, regen on the tanks, and Cure 1/2 on the OT until the add is dead, sch heals MT during this time. Medica 2 in the center, orb management and MT heals, sch dps's. Go into Megaflare, I medica 2, everyone stacks, I cure 3 for rage, let that medica roll, OT gets add, shroud happens to be up for us then, I use it here. Same thing as before. Last megaflare and add for us, I do the same process, this time my OT uses a few more CD's to include beserk to make sure he has aggro here. Kill that add, megaflare, rage, stack in the center find Bahamut, while cure 3 drops on everyone. Sprint run from DB.
Phase 3. I solo heal this up to the Sin. We are still working on the add to DB transition.
I hope this helps. Everyone should have 6.2k hp going into this fight in my opinion. This allows anyone to live with two orb stacks during the rage. As a whm, I get this from having my zodiac, chest/legs i130. My head, belt, and boots are i120, right side is i130 other than one of my rings. I use the triple cream coffee HQ to get as much vit as I can. Currently I am at 6250 hp and 5400 mana buffed. I have the lowest hp in our group. We do have a monk and warrior so this does help with the outgoing damage. I never need ballad if everyone plays correctly and executes mechanics appropriately.

About to start progressing on this turn ourselves, so these tips are extremely helpful (I main WHM). Thank you Sumimi for making this thread, and everyone else that has contributed.

Well, if you can't afford crafted accessories with just vitality melds. That works I guess(?) You need at least 6100 HP if there's a monk, 6200 HP if there isn't for Turn 13. I recommend having 6200 HP regardless of having a monk there or not, however. During phase 3 Storm's Path and Dragon Kick can't be applied so you'll have to survive on just succor and perhaps sacred soil. But you don't want to bet on the off chance on not getting that succor while practicing that phase. It may or may not lead to your death and another wasted attempt, which could have been spend on practicing that phase.
If it helps, here's a HP formula for you to work with to calculate the minimum about of vitality you need:
HP = 14,501vit - 1144,6
To determine your vitality without being in a party and without food, do it like this:
Current vitality * 1,03 + food vitality value (assuming you get the maximum out of it) = actual vitality for encounter
So let's say you have 400 base vitality. 400 * 1,03 = 412 vitality. Food gives you 20 vitality so you'll have 432 vitality then. Apply formula and you'll have 4830 HP.
PS: I've done T13 with my alt as white mage. Cure 1 heals for 1330 and was totally doable without overcompensating by burning extra MP.
We've cleared t13 3 weeks now and this is what I tend to do:
p1:
-Avoid Cure2:s unless mt drops below 4k from flatten.
-DS-Medi2 first megas, Medi2 the second megas, DS-Medi1 the 3rd (we tend to push the boss right after these), Cure3 to top everyone for Giga (I also use macros for Stack Up for them deepsers).
-After megas, I cure1 once, run behind boss if i'm not there already, Skin the mt, time my Cure1:s for Flat-breaths.
-Regen for ~90-100% on mt.
p2:
-I avoid Medi2 on transition because of the same reasons, i.e. getting slapped by the add. Though I do use it here. When boss does single-Breath, add should be dead and everyone running to boss, this is where I go middle'ish, DS-Medi2. This ensures it runs out before next add pops.
-Again I try to avoid Cure2 (unless Freecure). I'm also on mt while sch handles ot.
-We all have at least 6,2k, some even 6,7k hp to survive properly 2stack-Rage.
-During megas here, I avoid the ground-circles, stand enough close to the stack (if I'm not stack-megaed) for single Medi1, then spam my Stack Up macro for a Cure3 to top everyone for Rage.
-I let the sch handle mt during megas, mostly just pre-Skinning when mega is being cast.
p3:
-After Giga I pop Cure3 + Medi1 (or another Cure3 if needed) when we stack at middle. Be prepared for Bene if stackmegaed target gets any dmg.
-First adds sch deeps, I pop PoM and throw some sweet 1sec Cure1:s around (with Selene buff too). Tank can take 4k~ hits here so be prepared.
-I use Regen too here so that the small adds come to me when they pop (also using PoM to Skin myself quickly). Otherwise I dont tend to use Regen/Medi2 when new adds are popping.
-For Sins, I count to about 12s (or about 5-6 cures) to prepare for Evil Eye, then Cure3+Medi1 in preparation for Dives. Again be ready with Bene.
-As Storm spawns, it's easy to time Virus. When the next add after Storm pops, throw Virus on Storm and start Skinning. Should be in time for the big hit.
-I let sch handle the ot here and when the last add pops (think it was Pain), I'm free to throw some DS-Medi2.
Guess that should get you started.

I has an update!
We got to P3 last night, and I handed our OT a rotation so he could grab the adds with better snap aggro. This meant that I was free to use Medica II > Cure 3 for Megaflare P1, and Medica II > Cure 3 for Megaflare > Rage.
I also asked to try solo healing P1 so we can remove the 3rd flatten (and we succeeded). The Sch uses Selene for P1 as well now. At the beginning I entered P2 with < 200mp (eep), and now I'm probably in the range of 2k mp. With some gear, I can probably drop more Cure 2 spams for Cure 1.
It's moving along, I hope to get through half of P3 next week (if not the whole thing).
