I'd switch up your CDs. For one, you aren't guaranteed to have Titan on 3rd MB each add phase. If you do, you got bad DPS (or someone dead), which shouldn't be happening that often. Most likely what you are dieing to is healers falling to far behind in add/bomb phase. Keep in mind if you are getting to 3rd stack MB, it means adds are dieing slow, and OT is still taking damage, on top of healers moving for bombs/landslide/*and plumes if you are getting to 3rd stack*. Even good heals will be slowly falling behind at this point.
The best way to alleviate this is for your highest mitigation to be used during this period of movement/split heals. As far as *best* it is up to personal flavor in some ways. What I'd suggest is something like: First MB have IB + Foresight just as adds spawn. This is rather easy for heals to get you up, requires minimal CD, even just IB would be fine with Foresight for after damage or for second MB. Vengeance + Convalescence as Titan turns for landslide on group. This helps with bomb exploding on you for some recovery + MB after. Pop Featherfoot somewhere inbetween as well to help regulate incoming autoattacks and such. For 3rd MB, have IB + ToB. With the popping of convalescence and vengeance before, this keeps you topped or closer to topped for the 3rd MB if it comes, which is actually why you died. With full/near full health an IB is more then enough to survive the MB, and you have ToB for that extra HP to take the larger hit. At his point things will have settled down from adds, OT should be switching, and 1 healer not gaoled can get you up high enough to survive upheaval after.
Anyways, try it out, hopefully it helps. I personally like tanking adds on Warrior, I find the high threat gen to be worth it to secure add positioning. I usually see PLDs struggle with this a bit, where as their CDs help regulate the constant damage a little better for healers during the inevitable moving around of the add phase.
