Hmm.. well if we're taking instances of lag out of the equation...Batraal
Hydra
Chimera
Gogimera
Bees in SToQ
I'm sure there are more. I'm not sure if the abilities are technically instant death moves, but it seems like a lot of people are being one shot by them. The Hydra fight were I got one shotted out of the red killed me and monk in one shot.
Batraal: Can be sidestepped / avoided without much difficulty.
Hydra: I'm assuming you're talking about Triumvirate here so... this move is a frontal swipe and should only be hitting the tanks anyway (but it can be sidestepped as well) but as a DPS you should really never be hit by this unless you're attempting to have a staring contest with Hydra each fight.
Oh should probably mention that it also splits damage so.. the more people eating it at once = the less it hits for overall anyway.
Chimera: I'm guessing you're referring to The Ram's Voice, in most cases it's generally assumed this move is going to be silenced so chances are it wont go off anyway.
Gogimera: Hmm.. referring to Voice of the Dragon? this is avoidable based on range.
Bees: Well.. ask Nicolas Cage, if his career can survive them so can you.
jokes aside most if not all of these moves are preventable / dodge-able and even so with gear almost all of them are survivable should the first two fail.

Get more HP as in, "Go do Chimera with level 90 gear and survive a Ram's Voice that slipped through".Batrall, Hydra, Chimera, Gogimera and the bees are not a one hit death. They're a high dmg hit, get more HP, you'll be fine. Better yet, learn the mechanics of the fight and don't get hit by it. Well, the bees are hard to avoid I'll admit if not killed fast enough..
Now that I think about it... it's funny how the Cutter's Cry Chimera is better designed than Dhorme. On CC everyone's forced to move accordingly. On Coerthas, you hope the Bard/Monk/Paladin don't screw up, besides that, stand still until the red circle appears on your feet...
Last night I did the Sunken Temple of Quarn for the first time, and the party couldn't get past Teratotaur because of its Doom attack. The tank and one of the DDs either couldn't get out of the AoE, or couldn't find the square to remove it, and kept getting, well, doomed.
You aren't supposed to be able to get out of the AoE of Doom, but there is a bug that let's you do so.Last night I did the Sunken Temple of Quarn for the first time, and the party couldn't get past Teratotaur because of its Doom attack. If you are in AF gear then that is understandable... get full darklight and come back and tell me you are getting one shotted. Gear is an important factor...
The only one true one shot move I can think of is Doom in ToQ. Everything else sort of depends on group strategy and gear level.
You can't do anything about Doom, except by using an environmental mechanic to remove.
insta kill doesn't bother me, making it easier would be boring
Fantasy above reality.
I thought the labyrinth of qarn was the only death mechanic in the game. That doom spells doom on you.
