I don't think it's difficult. Personally, I think most people getting hit by it are just not good enough. When a static group of players from around the world can consistently do it without issue, then it's apparent that any lag problems are on players' end.

Someone is always suffering from routing issues in online games, but it becomes more apparent when there are stricter game mechanics and less servers spread across a very small geographical area. That's why services like WoWTunnels/Pingzapper exist. Whether or not it's SE's responsibility to take it up with ISPs is debatable, but if anyone has been watching their news feed, they'd know that's exactly what SE is trying to do.
