Technically, but 4.1x to 4.19 are considered under the "4.1" patch moniker, and we've always had that. And to be fair, I'd rather polished than buggy. :/

Yeah, the wait between 3.0 and 3.1 was like...5 months. I nearly quit for a bit when 3.1 hit since I was simply burned out. And yeah, they've been doing this since...forever. I don't see the issue either.
I have already lost faith in the developers when PotD came out and you could beat it with 4 dps.
The only compelling content is raids and ex trials, which both only reward gear that will 100% be obsolete in 1 to 2 patches. The whole structure of endgame is just rotten at this point. Endgame gear needs to at least last the whole expansion, and nerfs need to stop happening, to get people motivated to start putting in effort again.

They designed PotD this way on purpose, so that even dps could level quickly. It's still much harder to do without a healer and tank though at higher levels. The endgame gear is "obsolete" in 3-4, roughly. 1-2 is a inaccurate. Never if you use them for glamour. The structure is that of a modern MMO, aka the gear treadmill. You get more powerful as the expansion progresses. No amount of nerfs will encourage people who don't want to raid or do Ex modes...to do raids or Ex modes. People usually wait for the next expansion to do that content if they aren't interested in it when it's current. No nerf required, they simply come back OP. Echo will help, but if there's mechanics that need to be done, no amount of echo will save a group who refuses to do them. It all comes down to what people prefer to do, or not do. Not everything has to be about gear either.
You mean the game that started with more dungeons, a mythic dungeon system that kept them interesting, and a better reputation system to work toward that was more involved? with more pvp and better pvp? and world quests? and a raid with more difficulty levels? And has had multiple raids with each one being at an entirely different location instead of just being a continuation of boss rooms?
Yeah, FFXIV really has them beat in content currently doesn't it?

The horizontal progression over the continuous vertical version we have is an discussion hashed out quite a few times. That's an entirely different set of issues, and everyone seems to feel a bit differently on it.
The reality is, the current system is casual friendly. And there are more filthy casuals paying subs than hardcores who want it horizontal.
A full gearset is relatively easy to replace, the quicker turnover allows players to skip steps if they are behind, and the regular rewards keeps more people engaged. Horizontal progression and the way it rewards players grabs and holds fewer people that the faster gratification the treadmill does.
As for PotD, what are you talking about? PotD was meant as an alternative means to level grind, and ignoring the trinity meant it made the process faster. Or, is the issue that 4 DPS = easy content = boring and bad?
Depends on what you want from a mmo. By the way having loads of raid difficulties makes content get old very fast. I got very sick of doing several incarnations of the same bosses. Two difficulties is enough.
PotD is the worst piece of content ever added to the game, in my opinion. You just mindlessly kill things by pressing 1 button. No mechanics, no challenge, no nothing.
I'd rather do Diadem than PotD.
