Based on my understanding, it's 90 days since the campaign started. So, if you pay month by month, you should receive your mount by the end of September. People that received it early (like me), used things like 60 to 90 days time cards during the campaign period.

This would be correct. Basically, your subscription needed to have three months remaining as of July 1st to qualify for the first batch of Falcons sent out. Keep in mind, if you leveled a character on Omega for free time, that counts against the campaign. So you may need a time card to make up the difference.

This makes the most sense because I pay in 3 month cyrsta blocks. I had just came back and hadn't been playing for a year, maybe more but I would have had 3 months future sub around July 1. Sub lapse doesn't seem to have anything to do with it.
