I think he means, actions being represented by the UI as being ready, are not actually so. Thus misleading the player into attempting actions that are not ready.

Correct. This is the issue I mention in the OP.
To reword: I came to realize that the action-timer effects (both the time counting, and the dimming of the icon) on the action bar had stopped working. Hence, I was pressing icons for spells that appeared to be ready, but in reality were still in cool-off mode. This has nothing to do with the suppression of the action queue.
