Y'shtola mentioned that she cast the spell with the intention of saving Thancred, but I don't recall her ever stating if she actually succeeded at that. Would have to go back and check but I thought she said something went wrong causing her to transport herself instead of Thancred. Curious what happened to Minphilia given that she went back to check on them...
I suspect all Scions will come back altered in some way and receive a new outfit from Tataru.
Well it's not blatantly said what happened to Thancred, but remember they did confirm that his body was not within the rubble. We can deduce that Y'shtola's spell did indeed work to some extent. Also we don't know what Minphilia was going to do. Before she left she got a message from Hydaelyn. We can assume she's safe, I highly doubt Hydaelyn would send her to her death for little reason. What we do know is that Hydaelyn's plans for her were different than yours (the player character), and going your separate ways at that junction was necessary for you both to complete your tasks.
I have a feeling Matoya will give Y'shtola her eyes. If you do the Saro Roggo you'll see how deep Mato's love for Y'shtola is. Seems like a likely scenario.

Since Pipin explicitly said that they only found the bodies of Crystal Braves in the waterway, chances are the teleportation spell worked as intended on Thancred. The question now is where it dumped him.
One of the brooms mentions Mayotos crystal eye, during the Sarro quests. I took from it she had an artificial eye, but I was tired. It is just as likely to be an artifact.

I'm kind of hoping that Thancred and Yda turn up together, with a very strong reference to FFVIIs Honeybee Inn/Don Corneo storyline, with Thancred playing the role of Cloud...
