Here is a rotation that I'm kinda settling on. Please give feedbacks.

You can't get two IB's during Berserk with that rotation. Berserk is 8 GCD (20 seconds) long and you want to use two GCD on IB. So you need to get 5 stacks in 6 GCD's. Even if your rotation was possible (11 GCD's in 20 seconds) you waste three berserk GCD's on Heavy Swing's. My Suggested rotation would be:

Combo 1 (II)
Combo 2 (IV)
-Bloodbath {if you are in a health emergence}
Heavy Swing {before IB to get two stacks immediately after IB}
-Berserk (Infuriated)
IB
-Convalescence {if you need to replace loss of 15% heals}
Maim>SE or SS>BB (II)
-Vengence (III)
any Combo (Infuriated)
IB
Fracture or Heavy Swing
{+pacification}
-Brutal Swing/Mercy Stroke/Defense CD or any non-GCD up
