I am just going based off the comment. Did he insult me or my character? He insulted my character so while an assumption it is one grounded by the comment itself.
I have to add extra information to reach to the intent that their goal was to insult me.

Given how context is missing, I think it was wise not to report. Personally speaking I would have just ignored or said something akin to "As is your use of Caps Lock" if they where serious haha. That said if the abuse continued and was directed, I can see why you would report such a thing. Really no reason why you should yell that at someone, just keep your mouth shut if you do not like someones glam/character.2 days ago I queued for The Dusk Vigil (with my alt character, as a DPS). After completing the duty, just after I posted a "thank you" in the party chat and did a player commendation, half a second before I clicked "Exit", one of them obviously felt the need of typing this in the party chat: "UR CHARACTER IS HIDEOUS" (yes, in caps). I don't know if he meant me (my alt is a female Roegadyn) or if he meant someone else in the party. It doesn't even matter if I was the target of this. No one should be the target of insults, be it about race or outfit of their character.
I'm not trying to be "snow-flaky", but is this what FFXIV has become? I didn't see this kind of rudeness before in this game. Where did this behaviour come from?

What I think is kinda of moot cause my reasoning may not be the reason why they said. Getting impacted over how I perceive their comment without knowing their intent or reason seems weird no?

Idk, and good reason is subjective. See what I am saying just going based off "UR CHARACTER IS HIDEOUS" we cannot know the why outside they thought the character was ugly enough that they felt the need to say something.
That is the only intention or reason we can gather. We can speculate but that is also fueled by our own bias.
Just strikes me as weird to get bothered by something when you no way to know if it was aimed at you or not.
If you have a choice why choose the worse option?
What an absolute non-issue. Who cares what anyone else thinks of your character, so long as you yourself are happy with it?

In the ToS they even have clear examples of tone when responding to others in-game, with examples of violations and non-violations. Just outright saying someone's character is "hideous", while I was not able to find a similar or specific example, is very likely an offense simply by how it was said.
Additionally, a third party can find something offensive, report, and it should be considered a violation according to the ToS. There should even be examples, such as if there are 10 people, 9/10 of them can be fine with something, but if that last person isn't: it's a violation. It doesn't even matter if it is directed at them, if I recall.

Pretty sure the question was rhetorical, because there is no "good" reason to just go up to a random person you don't know and call them ugly. Even if you genuinely think that, it's extremely rude and only generates hostility between you. People do not like unsolicited negative comments about their appearance. This is such a basic fact of human social relations, that it beggars belief to consider the offending player did not understand that.
The exact wording of the remark, whether you let it bother you or not, if they're insulting you personally or something you've made, something you're wearing, etc., is really not the core of the issue. The issue is they want to say something that offends you. They don't need to say it. They want to say it. It's antisocial, combative behaviour. It's not meant to be constructive, or to open a thoughtful dialogue. It's fighting words. If Square Enix wants people to keep playing their game, they can't let this kind of interaction between players become the norm.
