If you are not interested in hearing a word from them anymore, blacklist. If you think they should get a warning or a ban for what they did, report. The first option is great for removing redundant personalities from your gaming experience and still allows you to use them as npcs for game content. Reporting works better for things like trade harassment done to prevent teleporting, as they won't be trading anyone if they are banned and can't log in.
The problem with giving it a universal pass because 'it's a one-time thing, you'll never see them again' is that certain people see that as free reign to go on and do it to dozens of different people.
So: I prefer to err on the side of reporting anyone who goes hostile for no discernible reason. Hardly takes more than a couple minutes, and whatever happens after that is entirely on them. If it truly was a one time thing, they'll get a warning at most.

You could have. Not the most egregious of insults but it's not like they are contributing anything of value to the community with commentary like that. You definitely should let it go after 2 days though. It really isnt worth it to pick your brain over something like this forever.2 days ago I queued for The Dusk Vigil (with my alt character, as a DPS). After completing the duty, just after I posted a "thank you" in the party chat and did a player commendation, half a second before I clicked "Exit", one of them obviously felt the need of typing this in the party chat: "UR CHARACTER IS HIDEOUS" (yes, in caps). I don't know if he meant me (my alt is a female Roegadyn) or if he meant someone else in the party. It doesn't even matter if I was the target of this. No one should be the target of insults, be it about race or outfit of their character.
I'm not trying to be "snow-flaky", but is this what FFXIV has become? I didn't see this kind of rudeness before in this game. Where did this behaviour come from?
After I had clicked Exit and realized how rude that player's text in the party chat was, I wasn't sure if I should report him. Should I have? Should I report next time such a thing happens?

Nah, just revel in their jealousy. If you run into them again, or someone like them, tell them that you have something to improve their outfit and send a trade request filled with glamour dispellers.
Are people really this thinskinned that they would report over this? It was some general comment about pixels. Just laugh at them and go about your day. Probably never see them again anyways. People seem to expect a giant hugbox everywhere now.

Comments like these are why I have friends who like playing MMOs but absolutely refuse to group with random players for dungeons. They got plenty of this in WoW back in the day and they are not going to tolerate it anymore. For every "I'm quitting FFXIV and here's why" post you see on the forums, there are many more players out there who just quietly leave or refuse to engage with MMOs in the first place. Telling them to "grow a thicker skin" or "it's not that bad of an insult" is not convincing.
It's also probably a large part of the reason why all the MSQ dungeons are made to be single-player with NPCs. People just don't want to deal with hostility and insults when they're supposed to be having fun.
You have to wonder, is this the first time that player has insulted others in chat? If they did, it obviously didn't come with any consequences. Why would they stop now?
