I don't think I have ever had the need to use it.
You can pretty much remove it from your hotbar.

Veteran healers don't care if we need to heal, but right now we don't. We want interesting things to do during the downtime other than a 30s dot and a single filler spell that hasn't changed from lvl 4 to lvl 90.
Dead DPS do no DPS. Raised DPS do 25/50% lower DPS. Do the mechanics and don't stand in bad stuff.
Other games expect basic competence, FFXIV is pleasantly surprised by it. Other games have toxic elitism. FFXIV has toxic casualism.[/LIST]
The question is: would the party have died if you had only used a plain Prognosis instead of EProg + Pepsis?
If the answer is no, then it was useless. It's like buffing Rapture with PI for more heal even if a plain Rapture would've done the trick. And I have honestly never encountered a single situation where Pepsis made the difference between life and death over a regular Prognosis. Not in low level content, not in highend content.
The only thing I like about Pepsis is that you can troll someone in a SGE/ SCH comp by completely deleting that juicy Spreadlo right before an aoe hits and watch the victim of choice die. Wouldn't recommend doing that outside of a static though. May also lead to the SCH yoinking you in retaliation for deleting that beautiful, megabuffed Spreadlo that was likely complete overkill but very satisfying to see.
