I'd love to help out. Can you walk me through a way to provide you with the necessary information?
Update on Immerse Gamepack: If you installed ANY version of the Immerse Gamepack by Embody, please install their latest version released today here - I just installed it and seems to be working fine so far. No audio issues or directx 11 crashes with weird sounds. If you still get stuttering/crashes/audio issues and already tried changing the performance settings to "Background Services", please try changing it back to "Programs" and see how it goes with this new release build.
I'll reply again whenever I get another audio issue or directx crash.
If it doesn't work for you, reply to Embody's twitter here - they're not square enix, so they're not actively reading the forums, and I think they were responsible for these issues. Let them know how it goes.
If you never installed any audio pack, and still get audio issues/crashes, try installing the immerse gamepack. There's a free trial going on, and it "might" solve the problem for you.
Also don't forget to lower your sample rate to CD quality just because.

Not everyone has a smartphone or a personal cell phone that they can use willy-nilly and install junk apps on to take pictures of their ear. This whole audio thing really smacks of Blizzard's Diablo mobile announcement. "You've all got cell phones, right?!"
I agree 100%. It's annoying and Square should've made this feature themselves. Also the full version is kinda expensive. Hopefully they fix the game with no need for 3rd party apps.
Yeah, installing this Immerse crapware is NOT an acceptable fix. I'm not saying it's what landed us here in the first place but I'm sure as hell thinking it. I'm not installing the free trial even as a temporary measure, let alone paying for it.
That said, I'm relieved to see Soken acknowledge the problems personally (guy deserves to have his work be appreciated), but it'd still be nice to see Squeenix acknowledge it on Lodestone or something for those of us who don't have the patience to dig through 50-page forum threads and twitter.
I finally gave in and submitted a bug report. Maybe if more of us do, they'll have to address the issue. It's been 3 weeks, I would like to know if they're at least working on it. I was doing a dungeon on bard earlier and the distortion got so bad that I just had to turn the sound completely off, which meant trying to field mechanics and procs solely by visuals. Lot harder than it sounds when you're used to hearing your little battle chimes and what not.
Yeah. I submitted two bug reports during 6.0, when problems first cropped up (for the first report) then how I fixed them (in the second report). Then when 6.01 completely broke sound, I submitted an in-depth bug report in game, along with all my system specs, description of the sound, and the things I had tried previously during 6.0 which had worked then but no longer worked in 6.01.
I think it's just a case of the devs having been completely caught off guard, seeing as how they thought 6.01 would fix the problem and not make it worse.
CPU: AMD FX-8350 63 °F
RAM: 16.0GB Dual-Channel DDR3 @ 933MHz (10-11-10-32)
Motherboard: MSI 990FXA-GD65 (MS-7640) (CPU 1) 105 °F
GPU: 2048MB ATI AMD Radeon R9 200 Series (XFX Pine Group) 101 °F
I almost mentioned ram speeds before, but I was a bit confused whether to say 933 or that times 2, since it's been a while and I forget how that works.
You can use Speccy to get the info free and easy with the timings just in case (the 4 numbers at the end above), small simple program I've been using for years. It should all be in the bios somewhere too, or other programs can probably get it, but this is just "open, copy and paste".
Speccy is here: https://www.ccleaner.com/speccy

Wow how long is SE going to not fix the Actual problem? vs giving us bad instructions on what did NOT fix the issue. Do they even recognize the issue as in KNOW what they BROKE!!
I am having the same issue and my friends kept saying it was my driver but it's updated. It's really annoying cuz it happens during story and interferes with audio comprehension. Combat with all the stuff going on gives a headache.
Let's get one thing straight, I'm not
