Sounds like you never played a mmo with a queue system. Usually works like this, to prevent these types of situations there is always rules in any kind of finder tool for example these are the most commonly used ones.
1. There is no ninjaing everyone gets a fair roll on loot. No player can set loot master / quarter master, a free roll system is used. Most games have restrictions on what people can roll based on their class and if they can use it.
2. You can kick people from the dungeon but it requires a majority vote to kick the person. Voting to kick people can not happen during combat or when there is loot in the treasure pool to be rolled on. Preventing people being kicked by someone because they want a rare item.
3. If someone has to leave or goes afk or is kicked by majority vote because they are causing a problem. The server will replace them with someone else in the queue and you will be on your way. People have to leave all the time for whatever reason but there is always people to replace them so its not that big of a deal.
No, i obviously haven't so why don't you name these games that use this great system you speak of, i'd very much like to try them out.
