FF14 doesn't like it if the audio device is "changed" while the game is running.
On the minimal end, it will stop the audio, on the extreme end it will crash the audio system, giving the xaudio crash you had the screen shot of.

If you are using a second audio device and you turn it on/off, plug/unplug it, or it has a hardware issue (bad cord, bad plug, bad power system, etc...) which causes it to "hiccup" it's active state, you can get the crash you are experiencing.
Try running without that device, just using the PC speakers or monitor speakers, and see if the issue persist.
Originally I was using a usb connected headset, but I replaced it with the mic / audio cord that plugs directly into the front of my tower ( mini-jacks ) .. after I had gotten the first error back a month ago is when I switched it out and didn't have a problem up until now.. super weird, im unsure if it has to do with my audio drivers but I am also experiencing the dx11 errors which I don't think are linked to the audio ones unless I am mistaken.
