Crap you're right. Either it was removed for spoilers, but then again why show a picture of him, or I'm merging two bit of info. But I did see it from some "official" source, but now I can't remember. I just remember it stating he's not a new trust, and the spelled his name without to apostrophe.
Also regardless of how he is labeled in game, the message I read was addressed to the players. Aside from trying to avoid spoilers, I don't know anyone who really refers to him as The Exarch, even before the dramatic unrevealed, lol
Must have been removed quite quickly if it was ever in the patch notes. I don't remember seeing it, and as I said before (and unless it's changed in-game in 5.1), it seems to be convention to keep calling him the Exarch officially.

Also if the information you saw was calling him "Graha" without the apostrophe (and without the "Tia"?), I doubt it was official - or if so, they didn't proofread it properly. You might be getting confused with something else.
Personally I alternate between referring to him as G'raha Tia or the Exarch (or occasionally mashing them together into G'raha Exarch*) depending on context. If I need to specify I'm talking about that older version of him, or I'm talking about the characters' understanding of the situation... eg. it's a technicality, but I would say that the WoL is friends with G'raha Tia, but the other Scions only know the Exarch.
It also feels like the 'correct' way of addressing him at this point? If you talk to him post-credits and call him G'raha Tia, he says how strange it is to be called by that name now. Maybe he'll get used to it again, but for now calling him Exarch just seems like the equivalent of referring to someone by the correct name/title. (For that matter, it's why I'll tend to use "G'raha Tia" instead of "G'raha" when I do call him that, unless it seems awkward in a casual mention - he seems to be referred to by full name more often than not, and definitely the three times our character has spoken his name in selectable dialogue. (It's one of those things I'm meaning to ask about next time they do a lore Q&A, but it seems that Sunseeker titles are treated more like part of their forename than a surname. It might be dropped in casual dialogue, but not formally.)
* "G'raha Exarch" is kind of technically in the spirit of canon formatting for Sunseeker names (Tia and Nunh being titles/ranks rather than personal names) but there is no canon basis for swapping in other custom titles like this. Still, it seemed logical enough to use in times where it does seem right to call him by name but I also want to specify I'm talking about the Exarch and not his younger self. I feel like it's closer to correct than the other option of "Exarch G'raha" at any rate.
