I think it's very possible it could be both.
I set up a few accounts for people a year ago due to them becoming very frustrated with the process. The accounts set up didn't have any problems. We all used Click and buy to purchase 1,000 crysta before billing began. I got receipts in my email, and the banks didn't say anything. I remember the registration was really way too confusing and brought back memories of my impatient teenage self trying to figure out how to set up billing in FFXI (which had me in tears T_T)... this was worse. Why they would chose this method is beyond me...
Is it possible that it's worse for people with more then one character or paying for larger amounts of crysta?
I only used the 30 day option and only bought enough crysta to pay for one month. Same with the other accounts. All of us only paid for one character. I am not sure if this makes a difference or not.
Before billing started I went into each FFXIV account, opened every option (payment method, period length) and selected and confirmed everything again to make sure it was all refreshed. I had to add a new card to the click and buy account, because the one a year ago expired. I tried the ultimatepay option but that one was terrible and wouldn't work...
I wish I could help more. :< I hope everyone gets a chance to play again. Don't give up. ^-^
