Not everyone runs in a static where those roles are guaranteed or the people playing them know how to supply you with MP when you need it. My experience with most BRDs was that they prioritize extending Foe's over support for the healer which isn't necessarily an invalid decision while MCHs are allergic to Refresh.
And you forgot BLM, which to me shows all the more you're talking about a raid environment which despite a lot of buzz on the forums is still run by a minority of the playerbase.
Oh. Derp. Yeah I missed that part. I guess it never really mattered to me since I didn't feel the clipping so many people complained about, maybe due to latency (shrug).And besides, the recast speed is the same.
BLM shouldn't manashift unless you have no other Refreshes in the party, it's the only job that suffers a DPS loss for it (stays in UI longer). Anyway even extending Foes, Refresh is still being played, you're still getting MP and you should always be casting. Communicate with your team so you don't overlap TA/Lucid and Refresh (since I'm assuming that's your problem).
When are they not? The problem is players just assume it's a cultural difference and devs say "well players do X and Y" when we do something entirely different.
It's not because JP players play drastically different than NA, it's just that the developers have too small of a sample size. They don't test properly and they're too slow to respond to feedback. We've known this since before 1.0 even launched.
The page I linked, players in that thread hate that change. But there are NA players that love the change and there are definitely JP players that love the change. My point is nationality has no bearing on how one plays the game or thinks.

Manashifting in Umbral Ice 3 gives you the mana back instantly, never had to wait before switching back.
You'll assume wrong. In general most of the situation you're presenting is very specific and relies on a lot of assumptions that don't translate well to common play from my experience.(since I'm assuming that's your problem).
It does not. You need to wait for the server tick, which can be instant, or it could be as much as 3 seconds. You've simply been lucky. And if it isn't your problem, then what is? Because Refresh is still mp, it doesn't magically give you more MP the later in the fight you are. If you aren't 100% mp, then it's effective mp.
This may effect Eureka in negative ways, though. relic armor was about half or so of the crystals you spent in there, and you didn't need to get level 20 to have a full relic armor set. If they don't balance this well, you could end up not needing to do any thing after hitting 40 save for the boss fate for the final step, cutting down on the time you spent there.
Not everyone is going in there to get every single relic. if they aren't careful it could make the lifespan of the content even shorter.
