Just did this last night as a healer. As someone who queue'd as a tank, that system is only good on a dps.
It gives you a warning, but doesn't stop you from queuing right on the click. If your queue is instant, you don't get a choice. It'd be much better if it gave a popup asking you to confirm you know you're going in with damaged gear.
[3) Put a repair NPC at the start of every dungeon. Lore wise, this makes 0 sense. Probably a better option still compared to #1 and #2.]
Except that they already put Novice NPCs outside the entrances to some of the dungeons. I'm sure they could find some way to make it lore friendly.
Also I messed up the quote tag, I'm on a phone and too lazy to fix it properly.
Forget lore. That screws over a bonus crafters get. You can queue for a dungeon anywhere for free and just do it. Unsynced too so you don't get penalty.[3) Put a repair NPC at the start of every dungeon. Lore wise, this makes 0 sense. Probably a better option still compared to #1 and #2.]

How about just let crafters repair other people's gear.
Vote kick and on to the next!
You can't do anything about people being jerks. They're gonna be jerks.
WHERE IS THIS KETTLE EVERYONE KEEPS INTRODUCING ME TO?
It depends on the situation which obviously differs on a case by case basis. However, in this specific situation, it's very doable to put in a system that prevents what happened in my situation and it's about time they started working on something, anything and no VTK is not a long-term solution to this problem because it should be happening to start with.
The whole system doens't need to change because one jerk did this. OP's post makes it very clear they were just doing this on purpose. There is already a system in place to warn of going in with broken gear. They obviously ignored that. This is why there is a vote-kick function. While I love the idea of being able to repair other people's gear, forcing this would be a pain in the end. Next time simply kick them and move on.
OK.. so lets look at this then.It depends on the situation which obviously differs on a case by case basis. Someone queued and either forgot or just did not want to repair their items. They got the warning but ignored it , possibly because since they were the healer it was an instaque.
You, reasonably politely asked about their gear state and were told you were elitist. ..... The fact you didn't kick them right there and then is admirable, but foolish. If someones reaction to you is first and foremost hostility and unwillingness to do something about it if possible, what can you really do? This isn't the system. This is the person. What would you do if someone queued with perfectly serviceable and repaired gear but swapped to broken stuff afterwards (for the challenge or whatever) ?
In my opinion the system is fine how it is. We have the tools to remove disruptive players and for someone who is actually concerned with being a contributing player the warnings are there. It's neither inconvenient nor expensive to hit a repair NPC reasonably often. They are everywhere.
What if I don't want to repair my gear to full before every queue? or even lower %? I have all my crafting at max, why should I not get the most out of my darkmatter and repair at 10% damage?
WHERE IS THIS KETTLE EVERYONE KEEPS INTRODUCING ME TO?
You do know that Dark Matter repairs add 100% durability to your gear, right? It doesn't set it to 100%. If your gear was at 10% durability, repairing it with Dark Matter would make it 110%.
