If you are using the macro on one of these panels, you have full control over who you select irregardless of other player actions. You control when you press a button, don't you? I mean, if you make a mouse over macro it works in multiple ways.

If your mouse is over their name in the party menu, you will heal them without having to click on them or their name - allowing you to maintain another target.
AND
If your mouse is over their player character, you will heal them without having to click on them or their name - allowing you to maintain another target.

So if I set a macro,

/ac "physick" <mo>
/ac "physick" <t>

and I have selected the tank and am healing them accordingly, but all of a sudden a BLM walks into my cursor which is sitting over on the right, I'll now be healing the BLM instead of the tank.
