1. Part of being a "good" team/player is knowing how to deal with this. Have a WAR? Push the healer off the edge and HG them at the bottom. Stunlock while your DPS kill theirs and bring your medals back. Got a PLD? Chainstun. BLM/WHM? Sleep.
2. 6+ stacks is stupidly easy to kill a tank through, even in tank stance. It can take around 30 seconds if the tank speads his CD's properly but with both dps on him, he should still go down pretty fast. The VUL will just be too high. As a DRG I can down a tank in around 3 fully buffed GCD's (w/ 1 or 2 oGCD's) if they have 6 or more stacks. A full rotation if they've got CD's up.
3. Melee LB is a "game changer". If the tank is hoarding medals (which he obviously shouldnt be) then he needs to save his CD's for surviving the remainder of the fight at the correct times. A good player can easily hit a stun during an LB and shut it down.
It just comes down to the quality of the player and if the team is aware enough to capitalise on the play.
