Anytime I play with a bad attitude. Just about every noob thing a BLM can possibly do will occur. I should instead wait to calm down before queuing for a dungeon.

Similarly, every time I tell some dumb person how to avoid a mechanic, or to stop sucking and dying to ______, I inevitably die to the same thing.
Law of maximum embarrassment or something.

Oh god yes .-.
I'm pretty good at explaining mechanics, so whenever I have new people in the party, I'm usually the one explaining everything.
Yesterday I went into Aurum Vale through df and got 3 new people, so when we got to the second boss, I made very obvious how important it is to not stay in his giant swing-around-attack and to not stand still when he targets you and shoots his eye beams in your direction.....only to proceed to get hit by EVERY SINGLE eye beam and not only getting closer than max range but close enough to instant-die on the big swing (as the healer).
Even worse, none of the new people failed, despite being in the fight for the first time while I did it hundreds of times for my relics -.-
I also failed badly on T12 just a bit before that AV run - when the Bennu phase was up, I had the great idea to triple flare the Bennus as BLM and then refusing to move from my comfy spot in the middle.
Tbh, it was new to me to have aggro on them so I rarely moved during the phase since movement is lost dps as a BLM - everyone was screaming at me to move in party chat which I didn't look at at all, so we ended up killing 3 Bennus in the same spot and had to deal with 3 buffed up big Bennus at once right after. No need to say that we wiped and I was nearly kicked from the party. I made it a rule since then to not bring MP refreshers into that fight to net get lured into using Flare too much x)

Woha, really? That's cool, I'll keep it in mind if I ever tank AV
Because I usually only play BLM/BRD/WHM though, all I can do is rely on melees and tanks to stun, and tbh, I'd rather not xD
Too many bad experiences relying on randoms to stun important moves in other dungeons & raids .-.
He's been stunnable forever. Everyone stand in melee, non-tanks dodge Glower, tank dodges Swipe, Eye of the Beholder misses everyone because it doesn't hit melee, stun the Swings. Easiest way to do the fight.

This happens to me, usually in the CT raids. I explain and stress how to avoid one hit kill mechanics. I end up derping and not avoiding the one hit kill mechanics. -_-
"So when KB rears up and the text shows a roar, run behind a comet immediately! Otherwise, you will die." *Gets so focused on healing something that the mechanic goes unnoticed till the last second....*
"make sure that when the floor turns red and blue that you alternate the colors red -> blue -> red -> blue every time it appears to change the debuffs on you." *Ends up with a second stack or in the rarer cases, third+death for not paying attention*
"Make sure that you turn away for Immortal Gaze - and if you don't and get Doom on you, run to a glowing pad to get it off before you die"
*Doesn't notice the Doom and dies*
