SoF EX would quickly become a ghost town unless SE gave it an actual reward that was comparable to the upped difficulty-beyond bragging rights on an internet gaming forum..

I'm sure you were just being facetious, but all the Ex battles do have tangible rewards of some kind so I don't know what you think would be different this time around.

Perhaps dungeon like Step of Faith or any dungeon which didn't give item reward never get the extreme version

There are a couple hard mode fights that don't give rewards that have Ex versions that do.
