Is it rage quitting if someone leaves before you do and you leave after?
Duty Finder Turn 5, after the 4th wipe due to misplaced fireballs/conflags despite very, very clear explanations of the mechanic. Sometimes you just have to cut your losses and run.
The one and only time I left a dungeon in the time FFXIV opened to now, was after a pugged tank was being rude to an FC mate. Not offering advice or constructive crit, just downright being rude and talking badly about them as if they weren't standing right there (probably trying to get the party to kick my FC mate by attempting to turn us against them/make him look bad) ...which was a pretty low move.
Party kicked the tank after that, instead (karma!) ....who popped right back in a few seconds later and proceeded to trash on the same person, right away. The three of us left, instead, knowing they'd just continue to return.
It was a collective thought - "You are an ass, and you won't go away."
I left the dungeon, but I wouldn't say it was a rage quit - more like escaping someone rude and abusing the 'Join Party in Progress' function.

I'm glad I'm not the only one. I'll do a dungeon/trial till I'm blue in the face, regardless the issue... I'm usually the one declining the vote abandons, as well. *doctor evil pinky* (...I can't help if I'm being honest with myself >_<)
Only times I'll willingly accept a vote abandon is if peoples stuff is broken/near breaking/everyone decided to take a break on good terms (but not abandoning due to 'oh em gee y u no kill dis right'), only 5 mins or so left and back at start from a wipe (Ultima HM, usually), half the party rage quit and aren't re-filling at all or other more important reasons.
What pisses me off most is when there's like, 30+ minutes on the clock, have a wipe or two and people want to leave (and then just sit and refuse to move). People need to man the hell up.

The only time I've ever ragequit was when I was leveling SCH. I was in Brayflox Story Mode. To make a long story short, the group I was with didn't know that Scholars can't use Leeches until level 40, and rather than try harder to avoid the final boss' poison breath, just started insulting me. I decided I don't have to put up with this sort of thing, and leave.
The funny part is that, 30 minutes later, I queue up again and get the fastest run of Brayflox SM that I'd seen up to that point (2.2).
Never ragequit or at least never be the first one leaving though.
Brayflox NM for an atma book. It's super late at night, i'm already pretty grouchy. Joined in-progress to find the other healer had already dropped. I see a level 50 War, a level 33 lancer, and a level 32 thrm (not blm). Tank proceeds to pull over half of the large open zone between the drake boss and the mudpuppy boss. Blows his cooldowns before he even as them grouped. I fight the good fight, but i'm levelsynced, so of course he dies. He asks how someone with T9 gear could fail so badly.

I tell him if he pulls like that again, i'm going to let him die. If he didn't like it they could kick me. He then proceeds to make rape allusions (ie. shhhhhh, if you just lay back and let it happen you might enjoy it.) Disgusting pig!
Anyway, rape jokes directed at me are always a deal breaker, so i was out. Waited 'til he started pulling again to leave just to get in that last little zinger. :P

Few days ago ST, was in group C. We had some problems on Scylla (wtf) but we pulled it off. Then on 2nd boss our BRD pulled, I don't know why but we were all inside and in places. A and B groups started calling C retards for pulling. Stopped healing right there and left right after boss died.
It takes a lot for me to want to quit, but the last time I did was when I was levelling Warrior.
We had a white mage in our party, and we were playing in Sastasha normal mode. Yes, an easy low-level dungeon, should be no problem right? Wrong. The WHM did nothing, sitting quietly content while my HP dropped ever closer to zero. When my HP finally hit double digits, lo and behold, a single cure was fired. Not a regular cure either, but the WHM basically SPAMMED Medica. So, not only a lower power heal, but they burned through all their MP just to recover me from a single encounter.
I figured they were new, and suggested politely they try using Cure II as it's better and heals for more. Moved onto the next mob group. Same again. Again, politely mentioning that they should really consider using some REAL heals or at least cast them sooner I went onto the next group.
Well, the third time just broke me. I told them simply that if it continues like this I wouldn't be sticking around. Their response? "Goodbye". So I quit.
I refuse to party with any healer who fails to do even the very basic level of curing and won't even take some polite advice.
