You didn't really mention anything about how/when to use Infuriate (the tank CDs are relatively obvious in how they should be used, but Infuriate is something of a special case), which, given the low CD, means that a lot of people are going to waste it by not using it. Not exactly sure how you feel about it, but I always try to use Infuriate immediately after I've just used a Wrath consumer with, potentially, a Heavy Swing thrown in between so as not to waste Wrath stack generation. Steel Cyclone>Infuriate>Steel Cyclone is an utterly disgusting opening volley against trash, and you can get a lot of mileage out of Inner Beast>Infuriate>HS>Inner Beast (gives you ~10 continuous seconds of the IB buff) on bosses. You could also use it at the start of a fight to enter with Unchained, but I prefer not to since it doesn't get the benefit of Maim; my opening volley against a boss is BBcombo>SEcombo>(Vengeance)>HS>(Unchained)>SS>(Berserk)>BB>(Internal Release)>BBcombo>SPcombo>Inner Beast; it frontloads enmity while getting as much as possible from the various damage buffs. Doing so will generate an obscene enmity cushion, allowing you to move into a debuff maintenance rotation (SP>SE) for a good long while without having to worry about anyone catching up to you (even other tanks).
On a random note, you should probably mention that delaying Berserk in order to stack it with Unchained provides the same benefit over time as using them independently because Unchained increases damage by the same amount you'd be delaying Berserk's usage by (also, always use Unchained *before* Berserk, so that you don't waste the Wrath stack that Berserk generates as well as not getting portions of Unchained wasted on Pacification). If you've got Internal Release, it syncs up perfectly and actually manages to provide a *slight* advantage over using the abilities independently.
