There is a Lalafell in the Ul'dahn Syndicate with much power an influence across Eorzea. His influence more or less undermines the current ruling family of Ul'dah.
You'll see his name crop up through the main story and some of the class quests.

There was a lot of controversy about F'lhaminn even in 1.0. She not only has round eyes, but also fangs, light skin, light fur, and a long tail...which are all Moonkeeper traits. It's only her name that says otherwise, which is why there have been debates on it. I highly doubt there would be such an oversight for a main character in FFXIV, which lead me to believe that F'lhaminn had one of each kind of parent, or at least a mix of each clan somewhere in the family line. Not sure if the devs ever cleared this issue up.
However, if this is true, then that would explain the lesser NPCs with mismatched names (since the lore of cross-breeding is a huge gray area, it may be canon that actual mixed races doesn't occur naturally and that all children are born as one race/clan or the other regardless of parents. This would mean the only way to distinguish them is by the name-race mismatch).
Indeed, I'll have to go back and check this out myself now.

Velox the thing with F'lhaminn is that she was designed before they finished actually selecting which trait was for each clan.
Currently, if you notice, all of F'lhalminn's traits are shared by both clans right now, if memory serves me.

True, and at this point I guess we can just justify it as a possible mixing anyway (for the sake of lore continuity) since the PC creation options violate the now-established lore as it stands. Other than the teeth and eyes for miqote, you can make either clan light/dark with any tail type/size and any name. Not complaining though, it adds spice to Eorzean life :P

Gotta remember that adventurers don't go by names, but by nicknames (remember what the guildkeepers say at the start? Adventurers use nicknames, nice trick from SE to essentially turn any weird name into canon names).
Also I don't quite thing the clans are that disparate in all features, they ARE same species, they're just close races. There are some continuity issues between 1.0 and 2.0 but those can be excused on change in storytelling direction, but I am more prone to believing that the current choices for PC creation do not violate the now-estabilished, lore, they are what define it.

Found her. "Detoh Moshroca". She stands next to Lanzenel on the main Tranquil platform. Wood Wailer, with a bone lance. She wears a mask so her eyes can't be made out but there's her name, and she has visible fangs when she's talking to you.
Also I got one of the quests last night where P'ebaloh, in Costa, mentions in passing, she was once a member of the Company of Heroes. Now it seems I'm finding strong and independent Miqo'te females in the story whenever I open a new door!

Ah, the Company of Heroes, the valiant men and women who first defeated Titan, without the power of the Echo too. After such heavy losses they had, the few remaining members took on peaceful jobs in Costa del Sol

Ah ok, I see who you were referencing now. Also I was mistaken earlier, it wasn't the regular leve NPC but the Serpent Leve NPC.
Here's a pic I grabbed
Like F'lhaminn, the name doesn't match the clan. Again, as Skies said before, it's probably just leftover continuity issues from the 1.0. I still think it would be more fun to use this convention for inter-clan children (since some of the naming rules for each race would make mixed names tricky). Naming a child with the opposite convention his/her physical appearance justifies makes things easier in the long run to pick out mixed relationships.

I just googled that, holy crap it's real. Also the wiki article is NSFW. Funny name for a infection, laughed when I first heard it in game.
