Everything else is definitely not real time. It's really easy to notice when casting spells because the casting bar is highly unreliable because of latency. Often you get interrupted for no reason because the server thinks you stopped moving after you started casting, the bar shows the wrong spell being cast or just completely miscalculates the length of the cast. It's pretty horrible.
