If SE has any sense they'll allow you ways to de/reactivate your code if your phone is lost. And tie it to your current security token.Indeed, I won't use this if I can't use my physical token alongside it. I've more chance of losing my phone than my token plus in 99% of cases the physical token will be more convenient. Pushing a button on my desk is easier than grabbing phone, unlocking it, opening up app and generating a code.
Your other complaints just sound really, really lazy.
