I'm guessing you're referring to a concept art that said 'Ishgard destroyed' while in fact it is only -an- 'Ishgardian fort' which has been destroyed. Yoshida said it was a translation error.

The funny thing is, I actually saw a screenshot with the line 'Ishgard destroyed' right under it.
Who translated it is irrelevant. But then again, I must be wrong, as you are always right. Oh, well. My bad.

From the original media pack during E3 it was misinterpreted because people thought it referred to Ishgard as a fortress rather than castle (the city). but then they clarified it was a fortress (different locale) and not the actual city of ishgard.
