There were symbols for Azeyma (Azeyma?) at a Meracydian chapel in Azys La, which I'm not sure if it was entirely an oversight (reusing textures), an oversight later made canon (reusing texture and rolling with it), or *ahem* Intended All Along(tm).

The implication seems to be that The Twelve have appeared or been sighted at important points in previous nations' histories that cults around them began to appear, and sightings of them may have led to unified depictions of them, especially if they've been sighted several times to reinforce said depictions. There are also some details that are off, particularly the Halone in the AST card looks much less armored than the one in Euphrosyne, and even the statues in Ishgard are juuust slightly off compared to what we've seen of her model.
