The exact same thing that people who purchase the expansion thinking they would get immediate access to Extreme, Unreal, Savage, Ultimate, Maps, Ishgard Restoration, Bozja, Eureka, Hilidbrand, Beast Tribes, ect. out of it. It's not the first- or last-time things will be gate kept by the story for lore reasons. I don't know why people keep acting so surprised and disappointed by something this game has been doing since ARR with new content? Like perhaps we should wait and see what the Island actually is/does before we start complaining about its unlock requirements. This just really feels like people are stuck thinking it was supposed to be some Animal Crossing fix/solution to current housing and it has been stated several times that it isn't and to lower your expectations on what the Island even is.

Oh if they purchased Endwalker, then they are golden, they just need to do some things with Eorzea first. *winks*

Do you really think someone will buy the expansion and appear out of nowhere in old Sharlyan?
