Teleporting can happen just because of network or FPS lag. You can fall through the map underground because of bugs (although I've only experienced this 2 or 3 times). An automatic countermeasure might get people who didn't do it intentionally.

The bans happen in waves, so even if they know about them they don't ban them all right away.
