I need help please someone anyone!

I can't get anything above one or two materia melds. The vast majority of the time the second attempt always shows as 40% and will always break if attempted. The once in a blue moon the item will survive the third attempt is 20% and over the two months trying will not go further than this point.
I've been using level 50+ items to meld on all of my level 50 crafting classes using grade 1 materia. I've upgraded all my crafting gear to be maxed out on each job's specific stat needs including materia melded to tools and equipment. I seem to be absolutely overqualified for melding and yet am completely unsuccessful at it beyond my control. It's extremely frustrating trying to get this achievement especially when my friends have earned it on their second go without effort at all.

I don't understand why I cannot meld more than two materia ever.

For unlucky person same as me, may do the following way:

use brass earrings and rings, rank 1 body and head gears to make r1 materia, only keep +1 - +3 str / mnd / pie / vit / dex

Cat 1 : Pie meld to ALC r48 primary tool
Cat 2 : STR / VIT meld to Armor r48 primary tool
Cat 3 : MND / DEX meld to weaver r48 primary tool

Once you get 40+ materia for one category, then you can start to meld. Provided that i hope I am luncky enough to get 1 success for 20 attemp....even system told me that success rate is 40%, I never got it......

If you successed to meld 2 materia into 1 gear, than store that gear, wait until you have 10+ gears melded 2 materia, than try to meld 3rd materia......just repeat until you got 10+ gears melded 4 materia, than try to meld the 5th one.....(I need 4 month to make 8 gears melding 4 materia....SHXX!)

But I've just experienced 10 continuous fail to meld the 4th materia.....with 25-30% success rate........Really never trust success rate from FFXIV, it is a lie. 20% success rate not means you will have 20 success for continuous 100 attempts.

Got this 3/3/12 5x MND I on a Woolen Beret. Got the 4th and 5th achievement on same item. Half Moon. Pure luck IMO. Good luck is all i can say.

Really never trust success rate from FFXIV, it is a lie.

Or any random number generator on this planet, including those jackpots in Casinos either.

So after buying all the tier 1 materia that was 1k or less (had some on retainer too), i made some lv48 items like raptor cullottes, pot helm,workboots etc and went and gathered some catalysts. I then sat down and started with the cullottes and got my crown the first try. Guess blowing up millions on double melds reset my luck >.>
