Sometimes, yes, they're looking for names, sometimes not. Of the things I have seen, it has been vague, with the GMs sometimes outright declining to disclose what was actually done. In the end, you can cry about conspiracies all you want, but you don't seem to disagree that these players should be made aware of what they'd done wrong. I just happen to believe it can be done, and should be done better.

"Many" is a vague word and since you really haven't brought up any specific complaint with proof, it gets harder and harder to believe.

For personal experience, I got brought once to the gaol for a report and they explained me what did I do to get the report (pulled mobs towards a leech in Pagos) and the resulting penalty (a warning). The only thing they didn't explain is who reported me.

So I'm really intrigued where did you get that from.

My references are as much non-evidence as your personal anecdote.

Otherwise, we do see threads complaining about a lack of transparency from GMs and rule-breaking, periodically. These usually include screenshots, etc. While I do think that some of those complaint threads are a little disingenuous, I also do think that there's some room for improvement in how these situations are handled, and will further acknowledge that, perhaps, some situations are beyond the scope of a general customer service agent which can lead to confusion on how to handle these issues.
...which circles us back to whether or not things can be improved, to which, again, I do believe that they can.

edit: Thinking about it, I believe the most recent ones I can distinctly remember came about when players were reporting party finder ads for RP venues, e or otherwise (but probably usually at least believing it was e). Although I'm sure there was at least a few more unrelated since, there have been a lot of disgruntled, "gaoled" players.
