All outfits should be gender neutral and just fit them to male or female bodies. ESO does it well.

Thancred and Y'shtola modish attire are like their own little bundle styled after that speciific character, each including a hairstyle.

People will find everything to complain about
It's difficult not to complain sometimes when this is a non-issue in many other MMOs as gear automatically changes to match the gender, and some of those are older than ffxiv.
XIV has a habit of handling things in convoluted ways, and this could be considered one of them. It's ridiculous that XIVs solution to using an already purchased item when you Fantasia is "hey, go buy another one for the other gender now, thanks!".

While I would say this would be reasonable for the Far Eastern Noble/Noblemwoman attire, but he also wants his Thancred outfit to be exchanged for the Y'shtola one, which is completely unreasonable because they aren't equivalent at all.
Yeah, that last bit was a bit of a stretch to me as well, but like you said there are many outfits on the shop that do have a normal male/female equivalent. Would be nice for them to implement some way to swap these without needing to buy a new set (either through the calamity NPC, or just fixing the gear to swap automatically in-game).
