Based on all I have read here I have contacted my ISP in the Netherlands (KPN) and they are contacting there technical department with the information about the packet loss through tata.ams03.atlas.cogentco.com (this one seems to be residing in an Amsterdam data center)

I also raised a ticket with Cogent support desk and called their support phone number in France and they acknowleged that they are looking into this issue of tata.ams03.atlas.cogentco.com already (so they are aware, dunno if the problem resides with them though).
First: Do not reinstall the game, this problem is not related to your PC or your installation.
- Mogstation (http://sqex.to/Msp) has same problem (login fails but also succeeds randomly)
- Main problems seems to connecting issues with HTTPS page for the oAuth login page (the login on the launcher works with an HTTPS page), but looks like all pages of square have problems
- It works for me 1 out of 5 times I try to login using the FFXIV launcher (I do get into the game if I try long enough)
- This forum also has problems with retrieve pages, as I also was not able to login on the forums first attempt
- I am from Netherlands, Rotterdam, Ziggo cable
- I know of one other person having the same issue from Finland
- Almost sure its a routing problem as going over the phone instead of fixed connection solves the problem (you get routed differently to the server). If this is true, there is a good chance that it is not on square-enix end but somewhere with the connections of hops between you and the server.
- People are reporting that login using a VPN or phone connection first works better. You can login first using phone/VPN. Once you see the 'play' button, switch to your normal internet and press play on the launcher.

Tried a little bit experiment: connecting my PS4 to my cell phone (Telekom) allowed me to login successfully. Switching to my Cable internet afterwards worked fine. There is something seriously wrong with some internet traffic (dns, https) with some providers and and SEs servers.
Still got loading issues with launcher, forums and lodestone. Tried every trick in the book, like resetting IE, flushing DNS, releasing and renewing IP, swapped from WiFi to cable to make sure it wasn't that, etc etc. Even ran WinMTR to secure.square-enix.com (as it was suggested in an earlier post) with no packet losses at all.
VPN is the only thing that works currently, as does the use VPN to login, then switch to own network before character select screen.
And I do not have any issues with any other website, service, launcher and whatnot. Only SE sites and services are having this issue at the moment.

I could get in after a while ( Germany ) did restart the Launcher many times , until i got the log in screen running ... took me maybe 2 hours , Hope SE will do something soon

They told me that according to their Technical Department the issue was "out of their hands" and said that they couldn't do anything. Hopefully your call will have more success then mine. For the moment I used the nice option by attaching my phone with USB to my PC and used the tethering/mobile hotspot thingy to bypass the authentication issue (launcher issue) as that is the only problem, reaching the authentication server which resides in Japan.
Did forget to check what route was taken from my mobile phone connection
Dunno why T-Mobile hotspot connection is working though, cannot care atm as I am playing ;-)
This happened to me as well, it has been almost two days, after I used VPN, I could log in without problems.
Happening to me too in Finland. Currently the forums and Mog Station are somewhat working unlike earlier, so I don't have to use VPN to navigate the forums for now. However, the launcher's connection errors still persist. I haven't used VPN to access the game, but I imagine it would temporarily fix the problem as others have said.
