Fun story here.
About two weeks ago, I was in E4S with my static. We had bashed our heads against Titan for about three hours, and we were debating on whether or not we should continue for an hour past our scheduled time to make more progress. Our Dragoon suddenly left the party after the next wipe, so we all left the instance, believing we were done for the day.
NOPE
A GM yanked him straight out of the instance and threw him directly into GM jail. So we were listening to our Dragoon freak out over Discord comms about what was happening, and some of the other members started giving him advice on what to say. We eventually settled on 'don't freaking lie about what might have happened'. When the GM showed up about 3 minutes later, said GM mentioned things that indicated they were actually watching us fight Titan, before making the decision to yank him out after that last wipe. Our Dragoon got released like 10 minutes later with a warning.
So, the story is that the Dragoon was in a random E4N party with pugs for his weekly clear, and apparently he 'greeded' too much (I assume this means taking damage from avoidable but survivable AoEs). This supposedly ticked off a healer in that party, who decided to rescue him into an AoE he couldn't survive, which killed him. Our Dragoon EXPLODED on him, and presumably the healer reported him after the fact. (Though, this sets a pretty dangerous precedent if this is how it all really went down - considering the healer griefed using an in-game skill, but chances are GMs can only see chat logs in reports and not the gameplay moments leading up to it.)
