It's a matter of preference sir.

I could ask you the same question. Instanced content sucks, why does anyone want it?

-Do people enjoy standing in lines?
    -Do people enjoy monotonous layouts?
    -Do people enjoy timers?
-Do people enjoy restrictions on how/when you can enter places?

I could go on, there are Positive and Negatives to both instanced and open world content. This is why I don't think all strong gear need to come from instanced events, thats just being narrow minded. There should be a balance of gear that is available in the open world, and through instanced events and then content that uses both.

IE: Going through a open area to reach the instanced boss fight. Beastmen strongholds, for example XI you usually had to go through the open world to reach your instanced events, XIV...you teleport to them in a matter of seconds.

Hmm. . . I've never had to stand in line for instanced content; the world of XIV atm is just as monotonous as the dungeon layouts; I actually wish they'd stop putting timers on every piece of content we get; and your last point is actually kinda analogous to respawn timers.

Anyway, you seem to actually be taking a somewhat moderate approach, which I agree with. I just like arguing sometimes. . .

Standing in lines? There are no lines in instances...they are lines in open world though. Monotonous layouts? That has nothing to do with the fundmentals of openworld vs instanced, you've confused me. If layouts are monotonous in their design that is an issue with design. Timers? Instances aren't typically timed there is a lot you can do with instance activities. Also, I'd "timers" are more of an issue with open world NMs.
Restrictions can be good or bad, having a minimum of 8 for a raid is BAD, people shouldbe able to do with whatever number they want. Rank restrictions though are GOOD. You have to work to experience content it can't be handed to you.

Anyway, please also remember that the issue is open world DUNGEONS, not open world NMs. There is nothing epic about competing for coffers all day on trash. I can udnerstand the epic feel of defeating an open world NM infront of everybody, i even said so in the OP. But in an open world dungeon not "everybody" is gonna make it through all the bullshit to see you defeat the boss so "epicness" VOID.

Somebody mentioned the idea that there should be a combination, that the entrance to an instance shouldn't be a plain aeth, it should be an area you have to progress through a bunch of mobs, for example imagine busting through the stronghold to get to the main NM, instead of being the main NM, that shold be an epic gatekeeper, defeat him and enter your instance, that sounds like a great idea. Matter of fact that was even a staple in WoW until they added queuing and porting directly to a dungeon.

Somebody also mentioned the diea of item pop open world NMs. That too is a great idea, let's do it. Again the competition is less vicious with a system like that. Waiting for a random timed mob to pop and having people squabble over them sucks.

The faction NMs are the perfect example of how open world NMs can be ideal, except for the fact that faction NMs blow right now and its really dumb how you pop them (by just doing random leves until you have enough faction points) Retarded. It would be better if there were other ways of collecting those points like grinding or doing some related dungeon or farming/trading in tiems. But only from leves?! No thanks...
