I play with a game pad and has cerrainly been a few occasions where I'm setting the boss as focus target or something before the fight starts and accidently lob an arrow at it.

Target boss squae up x. And if you hold x slightly to long the menu disaapears and you auto attack

Yikes, I wasn't even aware that's how gamepad controls worked with auto-attack. That's even worse...
