We talked about it in other threads, the duty finder one, as well as the actual bug report forum, as it started to happen right after patch 4.01 I think it was. And here is what we speculated, based on the experience we did have with different runs.
Sometimes you don't encounter this bug at all and your run will be very smooth, even if people don't do the mechanic correctly, which would mean moving during and at the end of the cast instead of just standing there. However, it occurs that and again it's just based on people's experiences, nothing said officially so please take it with a grain of salt, that if the raid doesn't deal enough dmg, this mechanic will kill you, if not done correctly. It also occurs that in higher dps groups, instead of skipping the mechanic, Ex death gets really bothered and throws that at you as well, when again this mechanic isn't probably executed.
Now obviously this is totally different from people just stacking and overlapping their frozen characters, because that will 1 shot the lesser geared people, but then again not what the OP is actually talking about, because that is not a bug, that's just not paying attention running people over. The bug is the one I mentioned above. Sometimes it just occurs based on Ex death's mood that day it seems lol and is absolutely random. But it more and more looks like the indications above. Sorry I can't prove it, but next time somebody runs it, pay some attention to it, see if you have high performance dps in your group or just slower then snail mail dps, and check if these problems occur.
If so, then that seems to be the "soft" dps check or some groups and the "slow your axx down" indicator for higher performance groups, and not an actual bug but something that was put into place intentionally. Maybe even as fun gimmick, we will never know till it's officially addressed or taken care off by hot fixes, if it's indeed a bug.
To avoid it, of course as usual, perform the mechanic successful and you won't have any issues. No matter how the party performs that day.
