Or the DPS are being idiots. I recently had a monk that was so concerned about doing the most DPS, they wouldn't move out of stacked AOE indicators. If they weren't one shot from it, I'd have to stop dpsing to heal them. Eventually it ticked me off so much that I just put Regen on them. The tank was using proper cool downs, but the monk was doing literally everything he could to rip hate off the tank. He died 7 times total in that run because he stood in AOE and ended up tanking his targeted mob instead of using AOE on big trash pulls.

"I wouldn't had died if the healer did their effing job." When literally all I could afford to do almost the entire run except for some bosses, was heal and sometimes throw a DoT out. He made the pulls take longer by not aoeing the whole pack. And put more strain on me by intentionally ripping the one mob off of the tank and standing in AOE.
