Only a few tries so far, but each second materia meld attempt was 29% even though I am far overranked for the item and the materia.
If I understand correctly, for the same grade of materia, the actual stat bonus does not affect your percent chance of success?2) How much a materia gives you does not effect the %
(Example: getting +1 str or +5 str doesn't effect the % outcome)
So far that seems correct. I had a bunch of Grade 1 STR materia in +1, +2 and +3 varieties, and each time there was the same 29% chance of success on the second meld.
The diminishing return curve on materia meld on NQ items occurs at around (Tier III Max -1) + (Tier III Max -1), the success percentage start to decrease in 1.00% instead of 0.50% for every increment of grade. The next limit of success percentage occurs at around (Tier IV Min +1) + (Tier IV Min +1), where success rate drops 2% for each increment of grade on a single materia. In other words, shii start to get real around Max T3 x2 meld, and you'll be crazy to meld max T4 +T4s atm.
Also, for melds beyond the second one, it seems the success ratio is folded in half. Thus in hypothesis if one were to attch five identical Tier III max materias to the same NQ item, the success ratio would be something like the following:
100% (1st) >> 22% >> 11% >> 5.5% >> 2.75%
This by no means is conclusive, we still need to gather up more data. What we do know atm is HQ base raises the base success percentage thus making higher grade melds more viable. Also if one wants to attach multiple materias to the same item, start with the lowest grade first, it'll help out the success rate.

Yes, I mean the within the single tier. I think STR materia tier I can only be between +1 STR and +5 STR. And I never saw a difference of % as long as the materia was the same tier.
I don't think this is conclusive at all. You're not factoring in what gear you're putting it on and what level it is. Maybe that is statistically correct for the piece you've been putting it on, but that may not be conclusive with all gear. You are correct that we need more data though. Felt Trousers (Red)
Base Materia: Ironman's Will III (STR+4 VIT+4)
M2 Materia: Ironman's Will III (STR+4 VIT+4), Ironman's Will III (STR+5 VIT+5), Ironman's Will IV (STR+7 VIT+7)
I got the T4 to meld finally (7th attempt) lol
And here's an example with M2 gear
Cobalt Sollerets (Red) +1
Base Materia: Bloodthirst III (HP+47) + Bloodthirst IV (HP+70)
M3 Materia: Bloodthirst III (HP+40), Bloodthirst III (HP+44)
Didn't attempt to M3 these, went 1/1 on the first attempt and I'm happy with them lol.
But as you can see, at least with higher tier materia / higher level gear, the probability of success does in fact change with grades within tiers.
I'm compiling further info and it is looking as if Meowy is correct. The amount the materia gives you CAN effect the % rate, but it doesn't always. ^^ I'll post new info soon.
