The one that sticks out to this day.. Was my first time tanking quarn normal.
I was just a baby war with zippo experience. On the FIRST pull YES the THREE bats. She does not heal me. So I'm popping every thing I have and yep I died. I'llpoint out she was wearing ddread gear so I'm guessing she was used to high level brick wall tanks where she can perma DPS. I died 2 more times all on easy pulls..
I have to say the bad taste from that still lingers..
No kidding. Was your healer AFK or something? When I get healers who pull stuff like that, I get out the cattle prod. If they don't shape up and get in gear (especially if they're over-geared for the dungeon) then I hit up an FC healer and tell them to prepare to Q in-prog, Cuz' someone is gonna be tasting boot shine in a minute.

No she wasn't afk. Just DPSing away. Still remember the name too, and know exactly what I will do if I ever get grouped with her again.
On my DRK, once I hit 60, I queued with my FC leader's also-fresh-to-60 AST into Fractal Continuum. We'd done Neverreap already with an excellent DRG and SMN who managed to whittle things down, all while being conscious of my sword from the Great Gubal Library, and the lack of enmity it could generate. But in Fractal, somehow, we got paired with two BRDs. Two BRDs who were friends, and who were somehow still wearing 115/120 gear. One of their bows was 145, the other was 133. And somehow, they met the iLvl minimum. They explained that they were new, and so before each pull, I would explain what was going to happen. They managed to dodge AoE with moderate success, but were still taking a lot of damage--and weren't doing much themselves, either. I tried dropping Grit a couple times to help out on bosses, but after watching the healer nearly OOM trying to keep me up (and taking a lashing in /fc), I realized I wasn't geared enough for that.
Finally, with 30 minutes left on the clock, we get to the last boss.
The Bards could literally not burn the adds down in time. Even with me Plunging to the adds to help out, and our healer staying in Cleric Stance, we couldn't burn them down. We tried for another 25 minutes, wiping repeatedly, before we finally vote-abandoned.
Another time on my PLD, once I breached iLvl 200, I was running Neverreap with an FC-mate who'd just gotten to 60 on his DRG, and wanted to start gearing. We went with our FC WHM, and we got paired with a level 60 Archer.
Not a Bard, an Archer. Equipped with what appeared to be a roleplaying or glamour set, mixed and matched from level 30-40 content.
It was the first time I ever spent an entire dungeon in Sword Oath, and we only just barely beat the soft enrage on the second boss. Waukkeon was a huge pain, too, because the phase where he swims around and runs people over lasted about 3 minutes each time it happened, and the Archer must have died at least four times.
He thanked us before he left, before we could comment anything about the fact that he had his wrong set on.
The worst part was that the gear that dropped was something nobody could use, and I'd already Eso capped for the week.
So...wait, who were you in this story? o.O
EDIT: Thought of another. Any time I get into content with an Arcanist or Summoner who keeps amber carbuncle or Titan-egi out (bonus points if they start pulling before you can get out a /p Hello!). Or any time a DRG thinks pulling ahead is fun.

Pad died on titan ex and I got blown off the edge lol bad times :/
There are so many, all from healers who stand around and forget to heal while not even dpsing to getting 2smn in a run who can`t aoe dps.
But the cake, or price perhaps, or pie or whatever, goes to a VIT speced WAR in A4 normal way back. He stood there in defiance and said to me (me being a STR speced WAR) "I will OT, you MT", I said that it was better if he MTd and I was OT seeing as my damage would be higher, it did not go over with him so I told him to change stance to dps stance if he wanted to OT, he did not, so pulled I did, him still in defiance, and of he was thrown aaaand he died since well... bad player? Or something, so second try and once more I tried to reason with the guy, and once more he ignored whatever I said. In the end the group disbanded.
Other than this story we have all the times healers want you to pull big and then don`t heal you, standing around doing very little, forcing you to use equilibrium to self-heal, destroying any change you have to use your own dps to speed up the run. Now I don`t mind if healers dps... but if it comes at the cost of a WAR tanks or any tanks dps so much that it lowers the groups overall damage output I just have to ask myself if it is really worth it.
Did the tank play bad and get you guys multiple wipes? If not, then... maybe he was just doing what he was used to and comfortable with. I know my horror stories are the exact opposite of yours. Too many tanks trying to speed run every damn thing and dying horribly several times over.
While I understand mass pulls can save time, most of the time it doesn't even save that much, and perhaps that's what the tank was used to, slow pulling. Nothing wrong with that either.
Just my thoughts.
