I can't count how many times I've zones in and had my stance robbed from me - and not realized it until the DRG or MNK starts pulling off me despite spamming my enmity combo.

I sometimes also forget entire sections of dungeons exist, which almost invariably makes me look like a dumbass when I get there:
DPS: "What about the pat?"
Me: "What pat?"
Pat: "ME! "
Me: "HOSHI-!"
Heals: "Sigh."

I also have a macro for marking targets that likes to misbehave, which makes me look like even more of a dumbass when I spend the first 20 seconds of a pull fixing the markings.

And sometimes, if I tank when I'm really tired, I'll revert to my old WoW keybinds: "Why isn't Charge working? Am I out of rag-...oh yeah."

And finally, those times when I'm left with only a 200-300 MP on my DRK as the boss shifts to a phase wherein they can't be attacked for several seconds/minutes. I OOM, Darkside falls off, and I quietly wish I could disappear.

Yeah..about that... I remember being MT for Titan HM and EX, and when the bombs popped there was always this lil sweet spot on the edge to stand that was close to original tanking spot, and caused you to miss the explosions. Yeah, lets just say I've accidently walked off the platform a few times lol

Trial Duty Roulette, Chrysalis, me as Paladin (MT).
Got some first timers, so I explained the fight a bit. Told the 2nd Tank to do OT, because my gear was better... .

Feeling strong and super professionell, I didnt realized how much I m still in my Machinist (and its crossbar settings). That means, I was using my machinist-opener-rotation, but as a Paladin. Well... I started the fight with 3 active defensive cooldowns, sprint and hollowed ground by accident. Sadly, I used hollowed ground with a macro which shouts my remaining life into the group channel.

No TP, hollowed ground wasted and the entire party knows it. Most shamefull day of my life xD

Hmmm prob the ultimate derp I did was first time OTing T7... I never used a single CD for adds... friend who was healing didn't have many good things to say...

Otherwise seen tons as well think best one was during drtrial - I was OTing and MT stands there in the red when Titan jumps... suffice it to say he dropped, I had to take over tanking the moment he went invulnerable due to changing to heart phase.

So, I was doing trial ruelette and got Garuda Hard, and I pulled Superna away from the party, but all of the DPS wiped and they yelled at me. So, I totally did something wrong! I have no idea what I messed up on exactly, but at least we finished the trail in more or less five minutes without wiping!

Back from Syrcus Tower era, I become tank on Alliance B. There's other tank on Alliance C that really want to hold boss aggro as he's tank from Alliance B by keep provoking the boss. I'm thinking like "WTF you want?!".

Did Garuda EX for the first time and it was in Duty Finder.
After the first 3 wipes a DPS said,

"At least three people don't know the strat."
I said, "Tell us then."
He proceeded to say some stuff and I trailed off because I didn't understand much of the terminology.
He finished with, "Don't worry about that bit thats the Tank's job."
Another DPS said, "Phew! Glad I don't have to deal with that!"
Then I said, "Hmm... whats that?"
Then he explained again and said something about not killing a Spiny, swapping and 2 stacks.
I said, "Whats a stack? How do we swap? What are we swapping?"
Another DPS said, "Tank sounds like fun..."

4 people left the party at that point.

In the end I told my FC what happened and we did it unsynced. They didn't even need the Spiny they just steamrolled the biach.

I almost wiped my party at 1% in Thordan Ex. I popped Hallowed for the Quaga, but it dropped off literally one second before Heavenly Heel landed and I had no other cooldowns left. I ate 18k dmg and died; Thordan turned and cleaved the party. The fight ended with our WAR killing Thordan by himself, at 1 hp with Holmgang up. Probably my worst derp to date.

This was back when i was leveling paladin. I just could not understand why shield lob and shield bash were unavailable. I checked twice that the level o the dungeon allowed for them. I apolegized to the party and we completed the dungeon without. It was only after i got out and took a good look at my gear i noticed my derp. It seems you need to have a shield equiped to do shield lob, how quaint. I forgot to reequip a shield after my last gear upgrade.

Second derp was when in the first few HW dungeons i was really having difficulty keeping aggro, it took an fc member in my party to point out that i should use circle of scorn. Which i didnt even realise i had or even had equiped to a hotbar. Boy did it make a difference in later dungeons.
