They existed, but were mostly limited to wealthy aristocrats, who had the money to pay for frequent meals of meats and other high-calorie foods. Everyone else was subsisting on vegetables and other low-calorie foods, while working they asses off to produce it. By medieval standards, there were not many fat people. Most non-aristocratic "fat" people were more likely than not very muscular beneath their girth (hee hee). Their body type just makes them look fat (see: male Roegadyn casters).
History lesson: the difficulty of getting fat in that time period is why so many works of art portrayed fat people as beautiful, since it was a difficult-to-attain ideal.
In what regard is it a PS3 limitation? Jiggle physics? Well, my avatar's boobs bounce just the same on PS4 as they did on PS3, so... who knows? I'd have to take a look at the programming complexity and see if the PS3 can handle... whatever difference it is you suggest it can't. Plus know the specs of the systems. And stuff.
